What are biostimulators?

Biostimulators work differently: they do not “replace” from the outside. Instead, they activate fibroblasts, the collagen-producing cells in the dermis, to become active again. The result is your own collagen, built up over weeks and months.

The difference in experience: filler results are immediately visible. Biostimulator results develop slowly and therefore usually look more natural and even. The skin improves structurally, not just superficially.

Biostimulators are not a replacement for fillers, but an additional or alternative strategy depending on anatomy, age, and treatment goals.

How do biostimulators work?

After injection, a biostimulator triggers a controlled biological response. Depending on the product, the injected material consists of calcium hydroxyapatite microspheres or poly-L-lactic acid particles. The body recognizes the particles as foreign, which attracts fibroblasts to the area and initiates collagen production around them.

Over the following weeks and months, the carrier substance is broken down by the body, while the newly created collagen remains. What remains is your own tissue, not a permanent foreign material.

This process takes time. Early improvements in skin texture and firmness often appear after four to six weeks. Full results are typically visible after three to six months and can last one to two years depending on product choice and individual biology.

When are biostimulators used?

Biostimulators are especially suitable when facial changes are less about isolated volume loss and more about a general decline in tissue quality.

Skin laxity: As collagen density decreases with age, skin loses firmness and definition. Biostimulators address this root cause directly.

Sculptra contains poly-L-lactic acid (PLLA), a synthetic, biocompatible polymer used in medicine for decades — originally in resorbable sutures. In aesthetic use, it acts as a pure biostimulator without an immediate volumizing effect.

Collagen build-up with Sculptra is particularly gradual. Typically, two to three treatment sessions are planned a few weeks apart. Results develop slowly and evenly, which has the advantage of looking very natural and often not being “noticed” by others as a treatment.

Sculptra is especially suitable for broad areas such as cheeks, temples, and the midface.

Choosing a biostimulator and selecting the right product, depends on multiple factors: skin condition, age, previous treatments, expectations, and what is anatomically appropriate.

At The Retreat Berlin, every biostimulator treatment begins with a thorough assessment. We use ultrasound to evaluate tissue layers: where volume is missing, at what depth, and whether a biostimulator or another modality is the right solution. This information cannot be reliably determined through visual inspection or palpation alone.

The treatment itself is performed by a physician. Dosage, injection depth, and distribution are planned individually, not based on a standardized protocol.

Another important aspect: biostimulators combine well with other treatments. In some cases, combining a biostimulator with targeted filler placement produces a better result than either approach alone. We discuss this openly during the consultation.
